may now initiate a distribution from IUOE Local 30 Annuity
Plan, if you are eligible, by calling 1-866-84UNION (848-6466)
Monday through Friday from 8:00 A.M.
to midnight ET. While
a Member Services Specialist will be able to help you
determine your specific distribution eligibility, you are
generally eligible to receive a distribution from you IUOE
Local 30 Annuity Plan if you are age 59 ½ or older and
retired or age 55 or older and receiving a Local 30 Pension,
are disabled or have not received an employer
contribution to your account for three consecutive
months.
When
you call about a distribution, you must enter your Social
Security number and press the # key, then enter your
personal identification number (PIN) and press the # key. To speak with a Member Services Representative, Press
“0”. Once it is determined that you are eligible for a
distribution, Fidelity will instruct the Local 30 Trust Fund
office to send you an application.
The application must be completed and returned to the
Local 30 Trust Fund office.
Fidelity will then be instructed by the Local 30
Trust Fund office to send you a distribution check or to
your designated rollover custodian. |
